Parents, please use the following form to communicate with the band boosters. This form is for ordering T-Shirts, indicating your intention to volunteer, make a donation to the band boosters, and pay school instrument rental fees.
If you indicate in the form that a payment is needed, it will ask if you wish to pay by check or debit/credit cards. Please indicate on the form which method you want to use, then use the Payment Instructions below to complete payment.

Check: If you wish to pay by check, please make it payable to EMS Band Boosters and send it with your student to school to place in the payment box in the band room.

The band program regularly relies on parent volunteers to help out with various events and opportunities throughout the season.
Some of these opportunities require a background check. APS now has 2 types of background checks:
Supervised
Unsupervised
Most of our volunteer opportunities that require background checks only need a Supervised background check - this is the classic background check from APS that can be submitted online by anybody and typically costs $12. Submit this background check here: https://www.aps.edu/police/background-and-fingerprinting/background-checks-for-prospective-volunteers
Manzano Mountain Retreat volunteers/chaperones will need the Unsupervised background check. This is a more involved check, requires a fingerprinting with APS, and takes a few weeks to complete. The cost is about $59. Additionally, this background check requires the principal to submit your information to APS to initiate the process. If you need this background check, please make sure you fill out the Unsupervised volunteer section of the parent form found above.
NOTE: This Unsupervised background check covers anything that would normally only need a Supervised background check, so if you are planning to volunteer with the Manzano Mountain Retreat trip, please submit for the Unsupervised background check, and you will be covered for any other volunteering opportunities.

APS Trip Permission Form (please request from Mr. Sorensen)
Students have one practice assignment EVERY WEEK. (The only exception is a week with a concert or field trip.) These involve several suggested exercises to practice, as well as at least one graded recording submitted on Smartmusic. Please check in with your students during the week to make sure they are working on them.
Students should refer their Google Classroom pages for these assignments.
Every student and band family should read the current version of the Band Handbook.
The EMS Band Concert attire has changed this year to be a little more gender neutral.
Concert Uniform:
White Shirt - Long or short sleeve. Preferably button up with a collar or other nice shirt. (NO T-Shirts)
Black Pants/Skirt/Dress (NO Jeans)
Black Socks and Shoes - If your shoes do not typically use socks i.e.: flats or heels, socks are not needed, but black stockings or flesh toned nylon stockings are recommended.
Casual Uniform:
EMS Band T Shirt (these can be purchased from the boosters if needed)
Blue Jeans
Has your child outgrown their concert attire from last year or do you have gently used black pants, black skirts, black dress shoes, boys' long-sleeved white shirts, or girls' white tops, or band shirts, you are ready to pass on? The Eisenhower Band Boosters are accepting donations of these items. Please deliver the items to the band room or send them to class with your child.
If your child is in need of clothing for upcoming performances or is unable to purchase a band shirt, please contact Mr. Sorensen.
